Our process is designed to be straightforward and effective, ensuring that rights holders can protect their work while also respecting the rights of our users.

Submitting a DMCA Notice

If you believe that your copyright has been infringed upon on our site, please send a notice to dmca@formsvirginia.com. Your notice should include the following information, as required by the DMCA:

  • A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.
  • Identification of the copyrighted work claimed to have been infringed.
  • Identification of the material that is claimed to be infringing, with enough detail so that we can locate it on our site.
  • Your contact information, including address, telephone number, and an email address.
  • A statement by you that you have a good faith belief that the use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner.

What Happens After a Notice Is Submitted?

Upon receiving a DMCA notice, Forms Virginia will take whatever action, in its sole discretion, it deems appropriate, including the removal of the challenged material from the website. We will notify the individual or entity that posted the allegedly infringing content, providing them with the DMCA notice submitted by the rights holder.

Counter-Notice

If you have received a notification of alleged infringement and you believe that your content was removed in error, you may send a counter-notice to dmca@formsvirginia.com. Your counter-notice should include:

  • Your physical or electronic signature.
  • Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled.
  • A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or a misidentification of the material.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the judicial district in which your address is located, or if your address is outside of the United States, for any judicial district in which Forms Virginia may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
